Support at Home is a government program created to help older Australians access subsidised home care services, equipment, and home modifications more easily.

It is designed to be fair, simple, and personal – especially for those who need it most – helping you stay healthy, independent, and connected to your community.

In addition to your ongoing Support at Home services, you may also be eligible to receive short-term support or equipment to meet your needs.

Assistive Technology and Home Modifications Scheme

The AT-HM funding covers home modifications or equipment tailored to your assessed needs, helping you stay safe and independent.

Assistive technology funding will also cover prescription from a health professional. Examples: mobility aids (walking sticks, walking frames, wheelchairs), toileting supports (bedpans, commodes) and bathing devices (shower chairs, bath boards). 

Home modifications include everyday improvements. In some circumstances, you may be approved for more significant works, such as a bathroom redesign to improve accessibility. 

Funding for both is available in three tiers: Low (under $500), Medium (up to $2,000), and High (up to $15,000 nominal). An aged care assessment will determine your eligibility.

Restorative Care Pathway

Up to 16 weeks of intensive allied health and nursing care to help you get back on your feet, regain independence, and prevent higher level of ongoing care. 

An aged care assessment will determine your eligibility for the Restorative Care Pathway. If you’re already receiving ongoing services, you may still qualify.

Funding: the pathway provides $6,000 to access allied health or nursing support to regain function and stay independent longer.

End-of-Life
Pathway

Extra in-home care funding for 12 weeks for people with three months or less to live, helping them remain at home if they wish. 

The End-of-Life Pathway provides higher levels of funding specifically for in-home services, such as personal care, domestic assistance and general nursing care to complement services available under state and territory-based palliative care schemes. 

To be eligible, a medical or nurse practitioner must estimate a life expectancy of three months or less, and you must have a score of 40 or below on the Australian-modified Karnofsky Performance Status (AKPS), which measures mobility and frailty.

How to get started 

If you're seeking government assistance for home care services, you'll need to apply for an in-person assessment with My Aged Care. Or, contact us to organise private services.

The first step for subsidised home care is to apply for an assessment via My Aged Care*. After you apply, an assessment organisation will call you to arrange a time to complete a face-to-face assessment. Remember, if you need home care urgently, you can start paying privately right away. *My Aged Care is the Australian Government’s system for accessing government-funded aged care services.

Assessments take place at your home or in hospital if needed, to discuss your care needs and goals. Once complete, you’ll be informed of the funding you are eligible for. After receiving the government letter with details for your approved services and funding level, contact your chosen provider.

  • For Support at Home funding, we work with you to create a Care Plan based on your approved service list and budget, with a schedule tailored to your individual needs. Once you approve your plan, services can begin and a care partner will stay in regular contact.
  • For other fundings, we help you choose the services that best suit your needs.

If you need services urgently, at WMQ you can pay privately for care services while you wait for your funding.

What you'll pay

You won’t need to pay anything if you’ve been approved to receive Clinical Care. For any Independence and Everyday Living services, you’ll contribute a percentage of the service cost, based on the income and assets assessment conducted by Services Australia.
