About the Referral Hub

The Brisbane South PHN Mental Health Referral Service provides information and support to GPs who wish to connect their patients with the most appropriate Brisbane South Primary Health Network (PHN) funded mental health service for their unique circumstance. 

When you refer, you will be recommending a level of service for your patient. Based on your service recommendations, we will connect your patient with the most appropriate Brisbane South PHN funded mental health service.

Service options

When you refer to the Brisbane South PHN Mental Health Referral Service, you will be recommending a level of service for your patient. Based on your service recommendations and other information on the referral form (such as whether the patient is in an identified priority group, or the patient's residential location in a region), we will connect your patient with the most appropriate Brisbane South PHN funded mental health service within a stepped care model of mental health.

The stepped care model aims to improve consumer access to appropriate mental health services, and is better able to respond to the holistic needs of a person experiencing mental health concerns in a compassionate way. The model emphasises a collaborative, person-centred approach to mental health care and continuity of care. Consumers will be supported throughout their mental health care journey, and are able to “step-up” or “step-down” their level of mental health care as needed.

Services recommendations include:

  • Low intensity/early intervention: mental health services for people with, or at risk of, a mental health condition. Includes groups, self-guided help, education, and peer support.
  • Mild-to-moderate: psychological therapies for people in under-serviced and/or hard to reach populations. Mental health clinician provides short-term psychological intervention for people unable to access services under the Medicare-funded Better Access Initiative.
  • Severe and complex: clinical care coordination for people with mental illness, provided by a mental health clinician.
  • Severe and complex (non-clinical): psychosocial support for people, which includes services designed to improve social skills, manage daily living needs and support independence.
  • headspace: mental health services for young people aged 12-25 years with or at risk of, a mental health condition. Includes service hubs in Capalaba, Inala, Meadowbrook and Woolloongabba.
  • Social and Emotional Wellbeing for Aboriginal and Torres Strait Islander peoples and their families.

Make a referral

Our dedicated team of trained staff are able to assist GPs to make appropriate and seamless referrals for their patients. Call our team today on 3717 7070, or follow the steps to making a referral below.

 Making a referral:

  1. Complete the referral form to refer your patient—the form is available as a fillable PDF or for upload to practice management software via the Brisbane South PHN website
  2. Send the referral form using fax (Fax: 07 3539 6444) or secure message via Medical Objects to the Referral Hub Brisbane South PHN Mental Health Referral Service WMQ (GW4106000JX)
  3. The Brisbane South PHN Mental Health Services Referral Service team will determine the best pathway for support through PHN-commissioned services or provide alternate options based on the referral information provided
  4. The Referral Service team will inform the GP on the determination and allocation of the referral to the identified support pathway
  5. The PHN-commissioned service provider will make contact with the individual to undertake an assessment and confirm eligibility, then inform the referring GP of the outcome.
