To be eligible to live at WesleyCare Cannon Hill, you will need NDIS funding that aligns with this specific housing and support model. This includes Specialist Disability Accommodation (SDA) funding for a High Physical Support apartment, designed for one resident in a two-bedroom setting.
You will also require Core Supports or Home and Living funding to access Supported Independent Living (SIL) depending on your individual needs.
We can also support people with alternative funding arrangements, such as the Transport Accident Commission (TAC) or the National Injury Insurance Scheme Queensland (NIISQ).
WesleyCare Cannon Hill provides shared supports at a 1:2 ratio with active overnight support, ensuring participants receive consistent assistance both day and night.
Our experienced team includes qualified Disability Support Workers (Cert III) and RN oversight, enabling us to deliver high-intensity supports for participants with complex needs.
Residents are also supported to engage in community participation activities, promoting independence and social connection. Together, this integrated model ensures each resident receives the right level of personalised care in a safe and supportive environment.
Yes. We provide both Specialist Disability Accommodation (SDA) and Supported Independent Living (SIL) supports through an integrated housing and support model.
Residents live in modern, purpose-built homes and receive SIL supports from our team under a separate agreement, helping them live as independently as possible while benefiting from coordinated housing and daily supports.
By providing both SDA and SIL, we can take a coordinated approach to supporting your needs. We are committed to transparency and will always explain your options clearly, including any potential conflicts of interest associated with providing both services, so you can make an informed choice.
