The next step in your journey to aged care is to make contact with My Aged Care, either by visiting their website or calling them on 1800 200 422.
My Aged Care is run by the Australian Government and acts as your gateway to aged care services in Australia. In order to apply for residential aged care, you will need to contact My Aged Care and answer a few questions to work out what kind of care will best work for you.
My Aged Care will then organise for you to have a face-to-face meeting with an assessor, also known as an ACAT (Aged Care Assessment Team) assessment. The assessor, usually a nurse or other health care professional, will chat with you about your current situation and the kind of care you need.
After the assessment, you will receive a letter that states the services that you are eligible for, and the assessor can also determine whether or not you are eligible for a government-subsidised placement in an aged care home. At Wesley Mission Queensland, we welcome both government-subsidised placements and fee-for-service placements.
Step 3 – Time to choose a home and apply
When you’re ready to make the move, it’s time to choose a home and apply for residency. Once you have made your choice, we will provide you with an application form to fill out and return to us.
