Having an active social life can bring great joy. We support you to maintain the important social connections that keep you active in your community, by assisting with shopping, recreation and centre-based social activities. 

 What kind of support can I receive?

We can support you to maintain your social life by offering:

Wesley Mission Queensland is an approved provider of Home Care Packages and the My Aged Care Commonwealth Home Support Programme and has been delivering these services for over 20 years.

Travel assistance

If you need transport support to help you attend appointments and other commitments, we can assist you through our community services. Our qualified transport team offers door-to-door services that make sure you get to where you need to be. If you are unable to access other forms of transport due to health and accessibility reasons, our services may be able to help you.

How much does it cost to receive these services?

If you are over 65 and / or living with a disability, you may be able to access these services as part of your Home Care Package, through the My Aged Care Commonwealth Home Support Programme or disability services. 

Alternatively, you can arrange to pay privately without any subsidies.

Social and community participation for under 65s

Participating in social and community activities is an important part of living a healthy lifestyle, but living with a disability can sometimes make it a harder to get involved. Whether you’re after new experiences and skills in a supportive, accessible environment, or are already connected but just need some help with transport, we offer a range of options for supported social and community engagement.

How much does it cost to receive these services?

If you are registered under the National Disability Insurance Scheme (NDIS) and have social and community participation in your plan, your services can be funded through core supports.  

Alternatively, you can arrange to pay privately on a fee-for-service basis.
